This form can be completed to donate by:
- cheque made payable to Parramatta Marist Old Boys’ Union Scholarship Fund;
- credit and debit cards – MasterCard, Visa or Amex; (do not send credit card details by email or uploaded online, surface mail only)
- direct deposit or EFT. Please include your surname in the reference field on the deposit slip or online form
The fields on the form can be completed using your computer and then printed and signed. You will need a PDF reader such as Adobe Acrobat Reader DC which can be downloaded for free at acrobat.adobe.com/au/en/acrobat/pdf-reader
Don’t forget to save the form as you go. The information is not sent anywhere. When you save the form check the answers are still there. When you are finished you can save the form and email it to the email on the form as long as credit card details are not included, or
print the form as a blank form and completed it by hand and send it by surface mail to:
